PASSPORTS

Starting 1 April 2010, manual or regular green passports will no longer be issued by the Embassy or the Department of Foreign Affairs. All regular green passports issued before 1 April 2010 will remain valid and will be honored by immigration authorities worldwide until their stated expiry date.

Filipinos whose passports have already expired or are due to expire in six months are encouraged to apply for a new passport.

Please note that personal appearance is required for passport applications. Personal appearance is necessary for verification of personal data and the signing and affixing of thumbprints on the application form. Exceptions may be made if the applicant is 8 years old and younger, 65 years and older, and mentally or physically incapacitated.

 


Schedule of personal appearance

Personal appearance is scheduled by appointment any time from Monday to Friday between 9:00AM and11:30AM in the morning, and between 1:00 PM and 3:00PM in the afternoon.

Note: Download and print  the passport application form. When filling out the form, do not attach photos or affix thumb prints. Send the duly-accomplished form in advance to the Embassy via fax (070-3560030), email (thehague@philembassy.nl) or post.

For forms sent by fax or email, please call the Embassy at 070-3604820 an hour or more after sending the accomplished form to schedule an appointment. For forms sent by post or mail, please call after two days to schedule an appointment.
 



Release of new passport

The passport processing time takes between 30 to 45 days from the time of application to the time of release of the new passport. Applicants are advised as to the approximate release date of their passport. Applicants may also choose to follow up on their applications with the Embassy after about 30 days.

Applicants should personally pick up the new passport at the Embassy and present the old passport for cancellation. The cancelled passport will be returned to the applicant.

A.  RENEWAL OF PASSPORT

1.  Duly accomplished passport application form (available at the Consular Section of the Embassy or can be downloaded here)

2.  Three (3) coloured photos of the applicant taken against a royal blue background. Applicant should be in decent attire with collar. Photo must be of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: Not less than 3 cm. The Embassy reserves the right not to accept photos that do not comply with specifications and international standards.

3.  Previous or current passport (submit photocopy of information page, visa page, and page bearing signature of signing officer)

Note:  For brown passport holders, please submit photocopies of the first three pages (and the pages showing latest Bureau of Immigration departure and arrival stamps). Please also present supporting documents with complete middle name, example: authenticated birth certificate issued on NSO security paper.

Basic Fee:  Euro 60 (Passport)

Others:  Euro 5 (photocopy services) *if necessary



B. REQUIREMENTS FOR FIRST-TIME PASSPORT APPLICANTS / CHILDREN BORN IN THE NETHERLANDS

Note:  A person born in the Netherlands is considered a Filipino citizen if one or both of his parents are Filipino citizens at the time of his/her birth.

Please check the information on passport renewal and submit the following:

1.  Duly accomplished passport application form

2.  Report of Birth (ROB) (for those who have not yet applied for an ROB, please check the requirements for an ROB) (present original ROB and submit photocopy)

3.  International Birth Certificate duly legalized by the Dutch Ministry of Foreign Affairs (present original Birth Certificate and submit photocopy)

4.  Three (3) coloured photos of the applicant taken against a royal blue background. Applicant should be in decent attire with collar. Photo must be of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: Not less than 3 cm. The Embassy reserves the right not to accept photos that do not comply with specifications and international standards.

5.  Passport of the parent(s) (submit photocopy of information page, visa page, and page bearing signature of signing officer)

6.  If parents are married, photocopy of Marriage Contract or Report of Marriage (ROM) (issued by Philippine Embassy or Philippine Consulate General in the country where the marriage ceremony took place)

Basic Fee:  Euro 60 (Passport)

Others:  Euro 5 (photocopy services) *documents need to be submitted in duplicate or triplicate

Applicable fees will apply for Report of Birth and Report of Marriage if these documents are applied for simultaneously.



C.  REQUIREMENTS FOR SEAFARERS

Please check the information on the MRP and submit the following:

1.  Duly accomplished passport application form

2.  Three (3) coloured photos of the applicant taken against a royal blue background. Applicant should be in decent attire with collar. Photo must be of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: Not less than 3 cm. The Embassy reserves the right not to accept photos that do not comply with specifications and international standards.

D.  ADDITIONAL REQUIREMENTS FOR CHANGE OF NAME IN PASSPORT

For applicants who wish to change their name in their passport due to marriage, divorce or widowhood, please check requirements/information on passport renewal and refer to the additional requirements below:

1.  From single name to MARRIED name

  • If married in the Philippines, Marriage Contract printed on NSO security paper and authenticated by the DFA. (present original and submit two photocopies)
  • If married in the Netherlands, Report of Marriage issued by the Philippine Embassy in The Hague (present original and submit one photocopy); or if married in another country other than the Philippines or the Netherlands, Report of Marriage issued by the Philippine Embassy or Consulate in the country where marriage took place. (present original and submit photocopy)

2.  From married name to SINGLE name

  • Birth Certificate on NSO security paper (present original and submit photocopy)
  • Marriage Contract with previous spouse on NSO security paper authenticated by the DFA or Report of Marriage issued by the Philippine Embassy in The Hague or by the Philippine Embassy or Consulate in the country where the marriage took place. (present original and submit photocopy)

        In case of DIVORCE:

  • Report of Divorce issued by the Philippine Embassy in The Hague (if divorced in the Netherlands) or by the Philippine Embassy or Consulate in the country where divorce was finalized. (present original and submit photocopy)

       In case of ANNULMENT OF MARRIAGE:

  • Certificate of Finality of Decision and Court Order/Decision authenticated by the DFA (present original and submit photocopy)
  • Amended Marriage Contract (with annotation of the annulment of marriage) on NSO security paper authenticated by the DFA (present original and submit photocopy)

        In case of WIDOWHOOD:

  • For those whose spouse died in the Philippines, Death Certificate issued on NSO security paper authenticated by the DFA or Court Order of presumptive death (in case husband had been missing for a number of years and has been presumed dead) authenticated by the DFA. (present original and submit photocopy)
  • For those whose spouse died outside the Philippines, original or certified true copy of the Death Certificate with English translation and legalized or authenticated by the Ministry of Foreign Affairs of the country where the Death Certificate was issued. (present original and submit photocopy)

Basic Fee:  Euro 60 (Passport)

Others:  Euro 5 (photocopy services) *documents need to be submitted in duplicate or triplicate

Applicable fees would apply for Report of Marriage if applied for simultaneously.



E.  LOST/DESTROYED/DAMAGED PASSPORTS

1.  Personal appearance is required for all applicants.

2.  Duly accomplished passport application form

3.  Sworn statement narrating the circumstances on how the passport was lost/destroyed or damaged, which will be notarized by the Embassy.

4.  Police Report for lost passport cases (original and photocopy)

5.  Three (3) coloured photos of the applicant taken against a royal blue background. Applicant should be in decent attire with collar. Photo must be of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: Not less than 3 cm. The Embassy reserves the right not to accept photos that do not comply with specifications and international standards.

6.  If available, photocopy of lost/destroyed/damaged passport

7.  Birth Certificate issued on NSO security paper and authenticated by the DFA. (present original and submit photocopy)

8.  If applicable, Marriage Contract issued on NSO security paper and authenticated by DFA or Report of Marriage (present original and submit photocopy)

9.  Applicants may also be required to submit one or more of the following supporting documents depending on the assessment of the application (e.g. discrepancies in personal information such as name or date of birth, etc):

  • Voter’s Registration Record
  • PRC / IBP ID
  • Baptismal Certificate
  • Land Title
  • Seaman’s Book
  • Driver’s License
  • Income Tax Return (Old)
  • Government Service Record
  • Old SSS-E1 form/or digitized SSS-ID
  • School Form 137 or Transcript of Records
  • Seaman’s Book
  • Certificate of Identity and Tribal Membership issued by the Office of the Muslim Affairs
  • Counseling Certificate issued by the Commission on Filipino Overseas (CFO)
  • Other documents that show full name and birth details of applicant and citizenship

(Present original and submit photocopy)

The Embassy shall require the applicant to pay the costs of verification if applicant is not able to present a copy of his/her passport.

The Embassy shall begin processing the application for an MRP only after the 15-day clearing period for verification with the Department of Foreign Affairs in Manila. If the applicant cannot wait this long, he/she may wish to apply for a Travel Document.

Basic Fees:  Euro 90 (Replacement of lost/destroyed/damaged passport)
